Nexus-Family Healing has an exciting opportunity for a Case Manager to join our team in our Kinship team in our St. Cloud office! This position will work specifically with relative/kinship caregivers.

Schedule/Location/Pay:

  • Location - On-site, covering the St. Cloud, MN area
  • Schedule - Monday-Friday Business Hours
  • Pay - $52,000-$56,000

Position Summary:

Our Case Manager provides support and services to relatives caring for youth in St. Cloud, MN and within the surrounding area. When a relative cares for a child or sibling group removed from their homes, the advantages are tremendous for the children - but the challenges these relatives face can often be large.


Primary responsibilities:

  • Networking with community to reach relatives caring for youth either formally or informally
  • Conduct targeted outreach to locate kinship families for youth
  • Connect relative/kinship families with needed services and information while providing direct support to the children and families as needed
  • Facilitates share & support groups for relative and kin caregivers.
  • Ensures regular ongoing communication with families
  • Complete Home studies and licensing for relative and kin foster care providers.
  • Recognize and value cultural differences in all aspects of work and service delivery

Required Education and Licensure:

  • Requires a Bachelor's degree in Social Work, Psychology or Human/Social Services related field.
  • Valid MN Driver's license and auto insurance coverage required.
  • Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges
  • Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values
  • Ability to pass all necessary background screening requirements per DHS rules and regulations
