Job description

GENERAL SUMMARY
Demonstrates knowledge of biopsychosocial factors related to the ages of the patient's served and incorporates knowledge base when developing treatment plan .Coordinates the care and services of patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; assumes a leadership role within the interdisciplinary team to achieve optimal clinical and resource outcomes; facilitates and coordinates the discharge planning process.
MINIMUM QUALIFICATION
Education, Knowledge, Skills and Abilities
BA in social work or related field and LSW. Master degree in Social Work from accredited school or university preferred. Must be able to work independently and as part of a multi-disciplinary team. Crisis Intervention skills, Therapeutic communication skills.
Required Length and Type of Experience
Two years of previous experience in a hospital or healthcare setting.
Required Licensure or Certification
LSW OR LISW by State ofOhio
Required Physical and Environmental Demands
Required to walk, stand, bend and sit. Routine and/or daily tasks or procedures occasionally expose them to blood and other potentially infectious materials. Must be able to remain calm and function clearly in a stressful potentially violent environment.
