Job Title: Social Services Director Skilled Nursing Facility

Location: Evansville, IN area
Position Type: Full-Time
Position Overview
A skilled nursing facility is seeking a compassionate and experienced Social Services Director to oversee the social services program and support the emotional, psychosocial, and discharge planning needs of residents and their families. The Social Services Director plays a vital role in ensuring residents receive comprehensive care that promotes dignity, independence, and overall well-being.
This individual will work closely with the interdisciplinary care team to advocate for residents, coordinate services, and ensure compliance with all regulatory requirements related to social services in a long-term care setting.
Key Responsibilities

  • Oversees and manages the facilitys social services program in accordance with federal, state, and facility regulations.
  • Conducts psychosocial assessments for residents upon admission and throughout their stay.
  • Develops and implements individualized care plans addressing residents emotional and social needs.
  • Provides counseling and support to residents and families during transitions, adjustments, and challenging situations.
  • Coordinates discharge planning to ensure safe and appropriate transitions to home or other care settings.
  • Assists residents and families with accessing community resources and support services.
  • Participates in care plan meetings and collaborates with nursing, therapy, and administrative staff.
  • Advocates for resident rights and helps resolve concerns or grievances.
  • Maintains accurate and timely documentation in compliance with regulatory standards.
  • Provides guidance and education to staff regarding residents psychosocial needs.
Qualifications

  • Bachelors degree in Social Work, Human Services, Psychology, or a related field preferred, or MSW preferred.
  • Previous long term care experience a plus
  • Prior experience in long-term care or skilled nursing facilities preferred.
  • Knowledge of CMS regulations, care planning, and discharge planning processes.
  • Strong communication, problem-solving, and interpersonal skills.
  • Ability to build relationships with residents, families, and interdisciplinary team members.
