Job description

Waters Edge Health and Rehabilitation Center is seeking a Social Service Assistant to join our social services department. As a Social Service Assistant, you will work closely with our residents to ensure quality care is provided and improve the lives of our residents. 

This is a full time position working Monday - Friday day hours.

Some duties may include:

  • Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs

  • Identifying individual social and emotional needs

  • Participating in the planning of the resident’s admission, community re-entry or transfer to another facility

  • Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

  • Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay

  • Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Job Qualifications:

  • Experience in long term care or SNF

  • Experience working with a diverse aging population

  • Social work setting experience is preferred
