Job description

Lutheran Village at Millers Grant is seeking a compassionate Full-Time Social Worker to join our mission-driven community. This position works Monday-Friday, 9:00 a.m. to 5:00 p.m., with participation in a weekend Manager-on-Duty rotation.

As a Social Worker, you will support residents and families through life's transitions by providing counseling, advocacy, care coordination, and access to resources that enhance quality of life and well-being. You'll work closely with residents, families, and the interdisciplinary team to ensure person-centered care while maintaining compliance with professional and regulatory standards.

What You’ll Do as an LVMG Social Work:

  • Provide psychosocial support to help residents adjust to illness, transitions, crisis situations, and end-of-life issues.
  • Identify and coordinate delivery of mental health services in collaboration with the Consulting Psychiatrist.
  • Offer supportive services to families, particularly during bereavement.
  • Serve as a resident advocate, ensuring compliance with resident rights under federal, state, and local regulations.
  • Coordinate and facilitate support groups for residents and families, as requested.
  • Interview residents/guardians to collect necessary documentation and obtain required signatures.
  • Explain facility services, care procedures, billing, and policies to residents and families.
  • Maintain timely and accurate documentation of all social work interventions and communications.
  • Ensure compliance with HIPAA regulations, Maryland COMAR 10.07.14 and 10.07.02, and professional Code of Ethics.
  • Assist with developing and updating social work policies and procedures.
  • Prepare and submit reports, recommendations, and plans of correction for survey deficiencies.
  • Attend and actively participate in Resident Review Team (RRT) meetings and clinical supervision (minimum 3 hours/month).
  • Assist in the planning, development, implementation, and evaluation of the Social Work program.
  • Support mission-driven care by maintaining cooperative relationships with residents, families, community services, and staff.
  • Maintain flexibility to work varied hours, including evenings and weekends as needed.
  • Share on-call responsibilities for Residential Living, including after-hours and weekend coverage.
  • Provide cross-coverage for social work services across all levels of residency, as directed.
  • May manage resident admissions, discharges, and transfers in accordance with facility policies and procedures.
  • Collaborate with the interdisciplinary team to assess and facilitate potential admissions as needed.
  • Maintain accurate bed availability, census records, and admission/discharge logs as required.
  • Obtain Maryland Social Work License within 90 days of employment.
  • Maintain current CPR certification.
  • Complete all required continuing education per the Maryland Board of Social Work Examiners.
  • Perform additional duties as assigned.

What You’ll Need as a Social Worker:

Education | Experience

  • Bachelor’s degree in social work from an accredited college or university, with completion of a program accredited by the Council on Social Work Education (CSWE). Master’s degree preferred.
  • Current, active Social Work license in good standing issued by the Maryland State Board of Social Work Examiners; licensure must be maintained as a condition of employment.
  • Demonstrated understanding of the aging process and the unique psychosocial needs of older adults.
  • Proven ability to work both independently and collaboratively within a team environment, demonstrating a cooperative, respectful, and compassionate approach to colleagues and resident care.
