Job Summary and Qualifications

The Transplant Social Worker will assist in the facilitation of care across the continuum for transplant patient care needs with a focus on the following:

  • The Transplant Social Worker will assist in the facilitation of care across the continuum for transplant patient care needs with a focus on the following:
  • Perform biopsychosocial assessment of the patient to meet their healthcare needs; obtain information from the patient based on assessment; diagnose, provide therapeutic interventions, and make referrals as appropriate; provide supportive counseling to aid adjustment to patient’s illness.
  • Evaluate the clinical needs of the patient, recommend treatment plans, develop and implement appropriate care plans to include discharge planning and follow-up care. 
  •  Collaboration with transplant team members, families, and community resources to enhance optimal patient outcomes is essential. · Maintain proper documentation. · Other duties as assigned.

 

EDUCATION/EXPERIENCE

  • What qualifications you will need:
    • Must have current Social Work License to practice in the State of Texas
    • Master of Social Work required, LCSW is preferred
    • Completion of a course of study with specialization in clinical practice OR served for at least 2 years as a social worker, 1 year of which was in a transplant program and has established consultation relationship with a qualified social worker.
    • Social Work transplant experience is desired. 
    • Must be able to demonstrate analytical and critical thinking abilities with proactive decision-making and negotiation skills
