Social Services Director

Location: Royal Oaks Nursing and Rehab | Urbandale, IA
Schedule: Full-Time | Monday–Friday

Position Overview

Royal Oaks Nursing and Rehab is seeking a compassionate and experienced Social Services Director to lead the Social Services Department in our skilled nursing facility/Long Term Care in Urbandale, IA. This leadership role serves as an advocate for residents’ social and emotional well-being and oversees all aspects of social services programming, compliance, and departmental operations.

The Social Services Director supports residents, families, and staff through periods of transition, adjustment, and crisis while ensuring regulatory compliance and high-quality resident-centered care.

Key Responsibilities
  • Develop comprehensive social assessments and individualized care plans, identifying medically related social and emotional needs, realistic goals, and actionable interventions

  • Actively participate in pre-admission and admissions processes

  • Serve as a compassionate liaison for residents and families, responding professionally and independently to concerns and needs

  • Accurately document residents’ behavioral, emotional, and psychosocial status in accordance with regulatory requirements

  • Provide individual and group support to residents and families during adjustment periods, crises, or special circumstances

  • Establish, coordinate, and expand a volunteer program to support resident social service needs

  • Orient new residents and families to the facility and assist with their transition to the community

Qualifications
  • Bachelor’s Degree in Social Work or a related field from an accredited institution

  • Knowledge of social service practices, procedures, laws, regulations, and guidelines governing long-term care

  • Strong communication, documentation, and organizational skills
