Job description

We are currently hiring for a part-time Social Worker position. The Social Worker shares the responsibility of identifying and providing appropriate interventions related to the social and emotional needs of our long-term care residents. Assists with arranging services to meet the needs of residents. Social Worker acts as a liaison between the resident, family, facility staff, and the community.

RESPONSIBILITIES:

  • Visits resident(s) upon admission to assess the social, emotional ane environmental needs of the resident(s) and monitors on an ongoing basis.

  • Prepares and maintains required documentation/assessments as regulated by local, state and federal agencies and as problems/situations occur.

  • Participates in ongoing evaluation of the residents' needs and goals through resident care plan meetings.

  • Monitors adherence to plan of care and flow of information. Provides continuing contact with resident/family to coordinate ongoing care. Provides crisis intervention, reassessment and uncovers problems as they occur in the delivery of services.

  • Provides counseling and emotional support in relation to health/social needs and plan of care. Provides ongoing emotional support to residents and families to enable them to cope with difficulties and proper utilization of services.

  • Acts as liaison between resident/family and relevant professionals, programs, agencies, etc. to help resident secure services needed and make preferences known.

  • Coordinates discharge planning. Participates in internal transfers throughout each community as the need arises.

  • Coordinates the placement of residents to alternative care facilities when necessary.

QUALIFICATIONS/REQUIREMENTS:

  • Bachelor's Degree in Social work or related field

  • One or two years previous experience in medical setting, preferably nursing home based

  • Skills needed in group process and communications, both verbally and written

  • Ability to interact with other professionals in a teamwork manner.
