Senior Social Worker – Flagstaff, AZ

Location: Flagstaff, AZ 86001
Schedule: Monday–Friday | 8:00 AM–4:00 PM | 40 hrs/week
Duration: 12-Month Contract | Possible Extension
Pay: $33/hr W2

Seeking an experienced Senior Social Worker to support veterans at the Arizona. Responsibilities include social services program coordination, case management, resident assessments, care planning, discharge planning, Medicare notices, VA referrals, staff supervision, and regulatory compliance.


Requirements:

  • Master’s Degree in Social Work (MSW) – Required
  • Active Arizona Social Work License – Required
  • 1+ year healthcare supervisory experience
  • Long-term care experience preferred/required
  • Level 1 Fingerprint Clearance Card required.
