Fort Behavioral Person-centered Therapy is seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) to join our dedicated team in a full-time remote position. This position is fully remote, allowing for flexible working arrangements while still engaging effectively with clients throughout the therapy process. You will be a part of a collaborative and supportive environment focused on continuous care and interprofessional team dynamics. Occasional evening or weekend hours may be required based on client needs, and you may also participate in on-call rotations to address urgent situations.

• Conduct comprehensive assessments to evaluate clients' psychosocial needs, focusing on emotional, mental, and social factors influencing their well-being.
• Develop and implement personalized care plans in collaboration with clients and their families, ensuring alignment with their specific goals and preferences.
• Provide counseling and emotional support for seniors facing life transitions, including health changes, bereavement, and family dynamics.
• Act as an advocate for clients’ rights, ensuring access to necessary community resources and services that meet their physical and emotional needs.
• Work closely with an interdisciplinary team comprising nurses, caregivers, and healthcare professionals to ensure comprehensive client care coordination.
• Conduct regular check-ins to monitor clients' progress, assess living conditions, and adjust care strategies as necessary to enhance safety and well-being.
• Facilitate effective communication among family members and clients, helping to resolve conflicts and promote understanding in care-related decisions.
• Educate clients and their families about relevant community resources, support groups, and available programs to enhance their social engagement and quality of life.
• Maintain accurate documentation of client assessments, care plans, progress notes, and all relevant interactions in compliance with legal and ethical standards.
• Stay updated on evolving practices, regulations, and policies within the social work field to uphold the highest standards of care and practice.

In this role, you will primarily focus on working with seniors and their families, understanding their unique challenges related to aging, health, and social connection. You will address issues such as mental health struggles stemming from isolation, grief, or chronic health conditions, aiming to empower them towards reclaiming their autonomy and emotional stability.

• A Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of relevant experience in social work, preferably in community settings, senior care, or mental health.
• Thorough understanding of social work principles, aging-related challenges, and awareness of available community resources.
• Proficient in assessment techniques and developing effective interventions that lead to improved client outcomes.

• Experience working within a Detox center or substance abuse rehabilitation context is a plus.
• Familiarity with telehealth platforms and remote therapy best practices.
• Background in advocating for seniors' rights and accessing community resources specifically designed for aging populations.

The salary range for this position is between USD 70,000 and 100,000 per year, commensurate with experience and qualifications.
