Job description

Position: Case Manager - Substance Abuse Res Unit
Regional Health Systems is Northwest Indiana's largest provider of community-based mental health care, primary health care and addiction treatment services. We offer adults and children-both uninsured and insured-integrated care through our community mental health center and federally qualified health clinic. Regional Health Systems is a member of the Regional Care Group.
Position Purpose:
To provide case management and ADL services to SMI, CA and SED clients and their families. These services are directed toward providing the skills and supports necessary to enable clients to function in the least restrictive environment with the highest possible level of functioning. Case managers provide assessment, service planning, coordination and linkage and monitoring functions.

Requirements:
