Southwestern Behavioral Healthcare is currently seeking a full-time Case Manager to add to our team of professionals.  This position will work in our Community Support Services department providing Assertive Community Treatment services to clients assigned to the team.

WHAT IS THIS POSITION RESPONSIBLE FOR?

  1. Provides face-to-face community based case management and skills training services from a recovery/person centered philosophy and orientation.            
  2. Evaluates individuals to determine psychiatric stability, functioning with regard to daily living skills, personal strengths and limitations, and environmental needs utilizing the ANSA assessment instrument.
  3. Provide community-based skills training and development services for adult client. Skills training and development will be determined based on individual/family’s identified needs
  4. Coordinates assessment results and translates the service needs into a Person-Centered Treatment Plan.
  5. Upon completing Provisional period of employment, participate in the CSS weekend/holiday coverage rotation.

WHAT'S REQUIRED FOR THIS POSITION?

  • Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice, or a related Human Service field.
  • Knowledge of available community resources is preferred.
  • Previous work with the SMI population is highly preferred.
  • Demonstrated proficiency with interpersonal and writing skills.
  • Excellent communication, time management, and organizational skills.
  • Must have valid driver's license, auto insurance that meets the Indiana State minimum coverage requirement, and qualify for Company's vehicle liability insurance.
  • Candidates must pass required background checks including county/state checks, sex offender registry check, and drug screen.
