Join a traveling Physical Therapist role in New Albany, IN (Home Health) where you will help patients recover from injuries and manage chronic conditions while experiencing life in a new location. Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA.
  • Active Indiana PT license with no restrictions required to start the assignment.
  • Minimum one year of physical therapy experience required.
  • Basic Life Support (BLS) certification required (AHA or ARC accepted).
  • Home health experience preferred.
  • Experience with HCHB and OASIS preferred.

Assignment Details
  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Days 0800-1600. Monday-Friday schedule reported as 8:00 AM-4:30 PM for typical coverage.
  • Weekly Hours: 40 hour guarantee indicated.
  • On-call/Weekend Requirement: One weekend per month required.
  • Coverage Area: Multiple counties including Clark, Floyd, Harrison, Jefferson, Scott, Washington, and Crawford (travel between patient homes expected). Mileage reimbursed between visits from first to last patient.
  • Primary responsibilities: Regular home visits, start-of-care, resumption-of-care, and recertification visits; documentation in HCHB and completion of OASIS assessments.
  • Education: Minimum Bachelor's Degree required.
