1. Monitor program participants and support adherence to program rules, expectations, and established safety protocols in accordance with organizational policies and procedures.
2. Build positive and professional relationships with participants to promote trust, engagement, cooperation, and a respectful program environment.
3. Provide appropriate guidance and support to participants and communicate immediate concerns or needs to the appropriate staff or supervisor.
4. Model and encourage respectful and appropriate behavior while supporting an inclusive and welcoming environment.
5. Conduct routine rounds as assigned to monitor the general safety, cleanliness, and orderly condition of facility and residential areas, and promptly report concerns to the appropriate personnel.
6. Accurately document and promptly report incidents, safety concerns, and potential or observed violations of program rules in accordance with established procedures.
7. Support site security by monitoring entrances, exits, and common areas in accordance with assigned responsibilities and established security procedures.
8. Perform light cleaning, organization, and other environmental support duties as assigned to help maintain a clean and welcoming environment.
9. Recognize and respond appropriately to participants who may be experiencing behavioral health, substance use, emotional distress, or other challenges, and promptly seek assistance when a situation exceeds the scope of the position.
10. Utilize approved de-escalation and conflict-resolution techniques consistent with training, organizational policies, and applicable procedures, and request assistance from appropriate personnel when necessary.
11. Provide supportive, nonjudgmental communication to participants regarding identified concerns and refer case management, clinical, or behavioral health needs to appropriately qualified staff.
12. Provide appropriate supportive communication and assist participants in connecting with available resources or services as directed by the Care Team or other authorized personnel.
13. Participate in multidisciplinary team meetings, as assigned, and provide relevant observations and updates regarding participant needs, concerns, incidents, and program operations while maintaining appropriate confidentiality.
14. Collaborate with other staff members to ensure consistent program operations and participant care.
15. Support program goals by contributing ideas and feedback to enhance service delivery.
16. Accurately complete and maintain records, logs, and incident reports in compliance with program policies.
17. Monitor and document participant activities, progress, and concerns for internal records and reporting purposes.
18. Communicate critical information to appropriate team members and supervisors in a timely manner.
19. Complete and submit general reports as required, ensuring accurate and timely documentation of activities, incidents, and compliance with organizational or program standards.
20. Demonstrate understanding and practice harm reduction methods
21. Assist with clients’ room checks and/or temperature control as needed.
22. Act as site lead during fire drills and real emergencies, and as lead in performing necessary evacuations.
23. On occasion will need to work an additional 4 hours after regular shift or 4 hours before the regular shift if situation warrants.
24. Provide reasonable assistance to participants using walkers, wheelchairs, or other mobility equipment when appropriate, within the scope of training and assigned responsibilities, and request additional assistance when needed.
25. Maintain shift reports and communicate with NENB Team when client issues arise
26. Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors.
27. Participate in required safety and compliance trainings and apply learned practices in daily work activities.
28. Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace.
29. Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards.
30. Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role.
31. Participate in JWCH’s customer service training called AIDET on an annual basis.

1. Demonstrate an understanding of AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) to apply in patient encounters or team discussions.
2. Utilize the AIDET communication framework to communicate with patients and team members in a manner that reduces patient anxiety, increases patient compliance, and improves clinical outcomes

1. Performs other duties and tasks as may be required or assigned.

1. Stand or walk constantly (for up to an entire shift) on various surfaces (tile, concrete, carpet).
2. Climb stairs, ramps, or ladders occasionally during shift.
3. Must be able to kneel, squat, bend for any duration of time and run if needed.
4. Occasionally bend/twist at waist/knees/neck to perform various duties, including lift or carry up to 50 lbs.
5. Constant use of hands, arms, and fingers in reaching/handling/grasping while using radio, phone, notepad, writing reports and other administrative tasks.
6. Work in various environments including adverse outdoor conditions such as cold, rain or heat.

1. Minimum high school diploma or equivalency, associate degree in social services preferred
2. One - two years’ experience in behavioral health or social services environment
3. Previous experience working with homeless persons or population is preferred. 
4. Strong ability to communicate with homeless individuals who are actively participating in substance abuse and other factors related to homelessness.
5. Bilingual (English/Spanish) preferred
6. Must be comfortable using computers to navigate web-based tools, complete online training, and utilize software for communication, reporting, and documentation purposes.
7. Experience handling confidential documents and detail-oriented.
8. Ability to use email and working knowledge of word processing (Microsoft Word) and spreadsheet programs (Excel).
9. Ability to communicate clearly verbally and in writing
10. Ability to assist with emergency and behavioral health personnel with aggressive patient situations.
