Job description

JOB PURPOSE:
The Site Development Manager is responsible for driving the growth of the Program of All-Inclusive Care for the Elderly (PACE) within a defined geographic territory. This role focuses on increasing enrollments opportunities through relationship-building with providers, hospitals, senior centers, IPAs, and community organizations. The Site Development Manager develops and executes strategies that align with PACE's mission of keeping frail seniors safe in the community, while ensuring compliance with state and federal regulations.
JOB RESPONSIBILITIES:
Market Growth & Strategy
  • Develop and implement territory-specific growth strategies to achieve or exceed PACE enrollment goals through the recruitment, development and expansion of potential referral sources, relationships and providers for the Account Managers to close the deal.
  • Identify high-potential ZIP codes and populations using demographic and market analysis.
  • Anticipate regulatory changes and incorporate them into outreach and market penetration strategies.

Referral Source Development
  • Build and strengthen relationships with hospitals, physicians, IPAs, senior centers, home-delivered meal providers, and other community organizations.
  • Educate referral sources on the benefits of PACE, ensuring they understand eligibility, program features, and referral processes.
  • Serve as the trusted point of contact for referral partners, ensuring smooth collaboration and communication until an Account manager is assigned to the account.

Community Engagement
  • Represent PACE at health fairs, community events, senior housing sites, and cultural gatherings to raise program visibility.
  • Develop culturally competent outreach strategies to engage diverse populations within the territory.
  • Partner with faith-based organizations, advocacy groups, and Community Boards to strengthen community presence.

Field Support
  • Work closely with Account Managers to ensure warm handoffs from potential sites to referral sources.
  • Monitor market changes and explore new opportunities in the assigned territory.
  • Track pipeline activity, forecast enrollments, and address barriers with potential partners.

Brand Development & Awareness
  • Collaborate with marketing to tailor campaigns to local needs and cultural groups.
  • Ensure consistent messaging about PACE benefits, including social, and supportive services.
  • Promote success stories to increase credibility with partners and the community.
  • Support all business lines under the CenterLight Healthcare systems i.e (TeamCare Medical, PATH, C2Q, Matter of Care and PACE).

Reporting & Compliance
  • Maintain accurate and timely reporting of outreach activities, referral pipelines, and Site Development progress.
  • Provide regular updates and market intelligence to leadership.
  • Ensure all outreach and referral development activities comply with DOH, CMS, and PACE regulations.

Schedule: 8:30AM - 5:30PM
Weekly Hours: 40
QUALIFICATIONS:
Education: Bachelor's degree or equivalent experience in Business, Marketing, Healthcare Administration, Social Work, or related field (Master's preferred).
Experience:
  • Minimum of five (5+) years of experience in Business Development, Provider Relations, or Territory Management in healthcare (PACE, MLTC, Medicare Advantage, or related programs strongly preferred).
  • Demonstrated success in developing provider and community partnerships leading to growth.
  • Strong knowledge of senior populations, long-term care, and healthcare delivery systems.
  • Excellent relationship-building, presentation, and negotiation skills.
  • Ability to manage multiple priorities and meet monthly/quarterly enrollment targets.

Language: Chinese Bilingual, (Mandarin or Cantonese) strongly preferred.

Physical Requirements
Individuals must be able to sustain specific physical requirements essential to the job. This includes, but is not limited to:
  • Standing - duration of up to 8 hours per day in the field/community
  • Sitting/Stationary positions - sedentary for 6-8 hours a day for consecutive hours/periods.
  • Lifting/Push/Pull - up to 50 pounds of equipment, baggage, supplies, and other items used in the job scope using OSHA guidelines, etc.
  • Bending/Squatting - have to be able to safely bend or squat to perform the essential functions under the scope of the job.
  • Stairs/Steps/Walking/Climbing - must be able to maneuver stairs safely, climb up/down and walk to access work areas. The position requires an individual to be able to travel and walk between sites/locations and work areas throughout the day.
  • Agility/Fine Motor Skills - Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools to complete essential job functions (i.e., typing, use of supplies, equipment, etc.)
  • Sight/Visual Requirements - must be able to read documentation, papers, orders, signs, etc., and accurately type/write documentation, etc.
  • Audio Hearing and Motor Skills (language) Requirements - must be able to listen attentively and document information from patients, community members, co-workers, clients, providers, etc., and intake information through audio processing with accuracy. In addition, one must be able to speak comfortably and clearly with language and motor skills for customers to understand an individual.
  • Cognitive Ability - Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job.

Salary Range (Min-Max):
$70,000.00 - $80,000.00
