Primary Purpose:

New Reach is seeking a full-time Clinical Case Worker for GROWW (Growing Resilience, Opportunity, and Wellness for Women), a program supporting head-of-household single mothers experiencing housing instability across prevention, crisis services, and housing programs. This role provides counseling support and integrated case management to help clients engage in mental health services, stabilize housing, and access essential resources such as food assistance, benefits, substance use treatment, and legal advocacy. The Clinical Case Worker will also support children in the household with behavioral health and educational needs. Services are voluntary and community-based, offered in the client's home (if housed) or another chosen location.

Essential Functions:
  • Assist with overseeing and managing referrals to GROWW, including but not limited to reviewing and determining eligibility requirements are met for referrals, entering referral data into required databases, and assigning cases with support from Senior Clinical Manager.
  • Assist clients in identifying unaddressed needs related to benefits, education, housing, medical, substance use, and any other identified goals
  • Complete initial and annual bio-psychosocial assessments to determine service needs
  • Complete mental health assessments with clients upon intake and every 60-90 days in collaboration with Senior Clinical Program Manager and Quality Assurance to evaluate program effectiveness.
  • Act as a liaison with local mental health providers and community resources
  • Work with family peer support specialists or designated staff to facilitate groups.
  • Provide clinical support to a caseload of single mother head of household women, linking them with mental health support or providing intermediate support under supervision of the Senior Clinical Program Manager.
  • Facilitate monthly case conference meetings to collaborate with case workers and peer support specialist
  • Utilize evidenced based practices of Trauma Informed Care, Person Centered Care Planning, Harm Reduction and Motivational Interviewing
  • Participate in case conferencing, team meetings, professional development and community meetings as necessary to represent the Clinical team, including Community Care Team (CCT), CAN Housing Case Conference, and other meetings when appropriate.
  • Provide client/landlord mediation and/or intervention and act as a liaison between clients and landlords to provide support for issues related to housing concerns (safety, sanitation, hoarding, etc.)
  • Maintain timely and accurate client files, including care plans, correspondence and case notes.
  • Assist with special projects and complete other duties as assigned to assure successful operations
  • Serve as a crucial part of the clinical team by actively participating in the Trauma Informed Care Committee, presenting clinical programs internally & externally, and providing a source of support and guidance for non-clinical case work staff.
Qualifications:
  • Bachelor's Degree Required (BSW or related Human Services)
  • Master's degree in human services field strongly preferred.
  • 2 or more years of experience in the provision of Social Services.
  • Highly motivated, creative with excellent interpersonal skills.
  • Ability to provide comprehensive, intensive services to at-risk populations, including acting as a strong advocate.
  • High level of organizational skills and ability to prioritize and manage multiple projects simultaneously.
  • Excellent written and verbal communication skills.
  • Excellent computer skills, including Microsoft Outlook, Word, Excel and Power Point.
  • Must have valid driver's license and access to own reliable transportation with proof of insurance for community-based visits
  • Bilingual/Bicultural preferred.
Expectations:
  • Regular schedule is Monday through Friday 9:00am to 4:00pm. May occasionally flex schedule to work evenings to meet the individual needs of clients
  • Have a solutions-based approach to presented problems.
  • Be supportive, engaging and encouraging to peers and staff.
  • Be adaptable to change and willing to take on more responsibility.
  • Maintain strict confidentiality concerning any client's or employees' personal information, living information, and/or HIV status
  • Attend regularly scheduled supervision
  • Mandated to report any witnessed or suspected child abuse/neglect to the Department of Children and Families (training provided)
