Your Role

The Director of Nursing and Practice Transformation leads the nursing and clinical support workforce across Near North Health's health center sites and owns the design of the clinical workflows those teams run. The role carries two responsibilities that belong together. The first is nursing practice: scope, competency, supervision, and standards of care. The second is practice transformation: the redesign of how care moves through a visit so that patients get quality care faster, with more dignity, and with fewer handoffs dropped along the way.
