Blue Cross & Blue Shield of Mississippi is seeking a RN Care Management Coordinator to serve as an essential liaison between patients and healthcare providers to review needs and ensure personalized care plans and provide appropriate coordinating services to improve patient outcomes. Specifically, this clinician works with the provider and members to ensure high quality, patient-centered care.

Primary responsibilities include:

  • Perform clinical assessments to develop a Plan of Care, oversee discharge planning, provide education, and establish overall coordination of care
  • Identify members and those requiring referral to Clinical Pharmacy, Quality Management, or Wellness Coaching to ensure high quality, patient-centered care.
  • Collaborate with clinical team on complex member cases to provide consistent care

This critical role requires clinical judgement, approachability and sensitivity to the needs of others with the ability to establish constructive and effective relationships along the way. Assessing complex situations, they navigate all available information to make informed decisions for the optimal level of care for a patient. Most importantly, this Nurse must excel in patient-centered communication demonstrating advocacy for the patients that individualize a comprehensive plan to meet their needs.

This position requires:

  • Bachelor's degree in nursing
  • RN licensure that is unrestricted in the state of Mississippi
  • Intermediate proficiency utilizing software in the Microsoft Office Suite (Excel, Word, PowerPoint)
  • Strong communication skills (oral/written) to serve as the liaison between patients, families and medical staff while actively listening and considering concerns.

Preferred include:

  • One year of clinical experience preferred.
  • Background in Hospital Case Management preferred.
