Silver Birch of Mishawaka is seeking a Wellness Nurse (LPN or RN) to join our Clinical Team. In this role, you’ll help support residents’ health, safety, and overall well-being through medication management, assessments, and daily clinical support. What You’ll Be Doing
  • Administer medications and treatments as ordered, with accurate and timely documentation
  • Complete resident assessments, including admissions, transfers, discharges, and change-in-condition reviews
  • Monitor resident health, including vital signs and post-incident follow-ups
  • Educate residents and families on care plans, treatments, and wellness needs
  • Support and guide QMAs and CNAs throughout each shift
  • Collaborate with providers and interdisciplinary teams to ensure residents receive the best possible care

What We’re Looking For
  • A high school diploma or GED
  • Two (2) or more years of experience working in senior living
  • A current LPN or RN license in good standing
  • Valid CPR, TB, and First Aid certifications
  • A nurse who is organized, compassionate, and comfortable working as part of a team
