Job description

Essential Functions:

? Measure and document member progress in accordance with Individual Service Plans (ISP).
? Maintain accurate, confidential member information in compliance with state and federal regulations.
? Provide life-skills to promote independence and personal growth.
? Follow all Civitan training policies and applicable laws regarding member information.
? Implement ISP plans and directed tasks as outlined by the PCSP (Person Centered Service Plan).
? Restroom support may be necessary.
? Engage with members individually and in groups to promote choice, dignity, and self-advocacy.
? Serve as a positive role model in communication, behavior, and professionalism.
? Support member health and safety, including assistance with mobility, hygiene, and supervision.
? Complete all required documentation with appropriate signatures.
? Administer medications as permitted and trained, following medical orders.
? Follow all program policies and procedures and participate in required trainings.
? Report incidents within required timeframes using approved forms.
? Maintain reliable attendance and adhere to scheduled work hours.
? Use of Technology with provided training.
? Perform other duties as assigned to support program operations.
