Job Description

In partnership with the Director of Assisted Living and Memory Care Director, Med Aides are responsible for assisting with resident medication delivery and management, providing activities of daily living, and observing, documenting, and reporting resident conditions.

Essential Duties & Responsibilities:

  • Preparing and assisting with medications as prescribed by Healthcare Providers to residents in Assisted Living and Memory Care.
  • Documenting medications given and/or not given in eMAR.
  • Communicate to the Resident, Healthcare Providers, Family/POA, and Director/Nurse regarding change in medication orders, refill challenges, or missed scheduled doses of medication.
  • Observing, reporting, and documenting changes in resident conditions in Yardi EHR progress notes and Nurse to Med Aide Communication Log.
  • Documenting and follow up on resident concerns on the End of Shift Report.
  • Medication re-ordering and receiving, so residents do not run out of medication and confirming medications are logged, properly labeled, and stored.
  • Notifying Director/Nurse of any medication discrepancies.
  • Providing first aid when needed and responding to medical emergencies.
  • Assisting with completing internal incident reports and data entry into Yardi EHR.
  • Documenting alert charting, once per shift, in Yardi EHR progress notes.
  • Documenting monthly weights and vital signs as needed in Yardi EHR.
  • Maintaining a clean and organized medication room, med carts, and medication overflow areas.
  • Completing a narcotic count with oncoming shift.
  • Destroying medications per policy.
  • Participating in weekly medication audits.
  • Responding to resident pendent calls as needed.
  • Assisting residents with ADLs according to the resident’s personalized service plan.
  • Working in collaboration with caregivers to provide quality resident care and recommend ways to improve quality of care.
  • Following infection control procedures.
  • Maintaining confidentiality of residents and family information.
  • Following SRG policies and procedures.
  • Completing monthly training assignments as scheduled or assigned by Director.
  • Other duties assigned.
Qualifications
  • CNA or experienced caregiver.
  • First Aid / CPR certified.
  • Completion of state regulatory certifications and on-going training requirements.
  • Six-month experience in related position.
  • Comply with licensing requirements.
  • Experience with Yardi, eMAR, Schedule Pop, ADP, or other relevant platforms.
