Job Description:
Job Summary
Ensures a seamless and efficient discharge experience by facilitating communication among the care team. Identifies and addresses barriers to discharge, and supports patients and families in understanding next steps. Monitors progress and follows up to promote safe transitions of care. Communicates effectively to optimize patient outcomes and satisfaction.
Essential Functions
• Serves as liaison between patient families/caregivers and care team to facilitate effective discharge communication.
• Identifies and mitigates barriers to discharge prior to discharge day.
• Verifies placement of equipment orders to ensure safe discharge.
• Communicates hospital follow-up and specialty appointments to patients/caregivers.
• Confirms new prescriptions are sent to the patient's pharmacy.
• Verifies discharge transportation arrangements.
• Notifies primary nurse when patient is ready for discharge, ensuring all orders, medications, and transportation are in place.
• Identifies high-risk patients for readmission and coordinates post-discharge communication with input from Case Managers.
• Performs other duties as assigned.
Education Requirement
High school diploma or equivalent experience
Experience Requirement
0-1 years of experience in a healthcare, clinical setting, or a relevant area
An equivalent combination of education and experience may be considered.
All experience must be paid and in the same related field.
Part-time and PRN experience will be prorated based on hours worked per week.
Volunteer work and internships for academic credit are not counted.
