Coverage Area

Responsibilities
  • Evaluate and treat patients in the home health setting
  • Develop and implement individualized plans of care
  • Provide therapeutic exercise, manual therapy, and functional training
  • Educate patients and caregivers
  • Complete documentation in the EMR system (HelloNote experience is a plus)

Qualifications
  • Active Georgia Physical Therapist license
  • Degree from an accredited Physical Therapy program
