Overview

Salary Range $16.00 Hourly Position Type Full Time Education Level 2 Year Degree

Description

POSITION SUMMARY: To assist the Program Manager in the daily oversight and coordinator of providing nutritionally balanced meals, nutrition education and nutrition risk screenings for Floridians 60 years and older. Services are provided at dining facilities to include senior centers and other community focal points.

Full Time Position- Benefits Eligible: Vacation, Sick, PTO, Employer Paid Health Benefits, 403b Retirement, 14 Company Paid Holidays, Pet Insurance, Basic $30,000 Life Insurance Policy, and Long-Term Disability (LTD). Public Service Loan Forgiveness Qualified Employer.

ESSENTIAL JOB FUNCTIONS:

  • Oversees client record keeping to include, but not limited to; assessments, reassessments and eligibility determination. Maintains client records on an ongoing basis to include data entry of all client information into relevant databases
  • Completes intake forms, refers clients to appropriate resources as needed
  • Responsible for coordinating and training of meal site staff/volunteers in maintaining health/safety standards and meeting state/federal guidelines
  • Provides monthly activity calendars for assigned for meal sites and nutrition education materials to all active clients
  • Monitors and projects weekly and monthly meal counts, ensuring proper compliance with all required paperwork for each meal site, i.e. meals served, meal orders, sign in sheets, temperature logs and all activities at each site
  • Responsible for collection of donations at all assigned meal sites
  • Develops meal delivery route sheets and occasionally assists with delivery of Home Delivered Meals and/or serving at meal sites
  • Assists Community Outreach Coordinator in the promotion of the program.
  • Assists with report preparation and preparation for all monitoring activities.
  • Performs calldowns to clients during an emergency/disaster.
  • Participates in regular safety, storm and fire drills
  • Maintains confidentiality in all aspects of client, staff and agency information
  • Maintains professionalism, a positive image, and effective working interactions with co-workers, clients, and all outside contacts within the community
  • Uses and follows the policies/procedures of You Thrive Florida, including but not limited to Personnel Policies, Occupational Health and Safety policies, payroll policies/practices, etc.
  • Attends and actively participates in training programs, staff meetings, and other meeting/trainings as assigned

Qualifications

JOB STANDARDS:

Education: Associate's Degree in Human Services related field from an accredited college or university preferred.

Experience: One (1) year experience working with elder population. An equivalent combination of education/experience may be considered.

Licenses & Certifications: Valid Florida driver's license and valid insurance. Subject to Federal, State and Local legal requirements/background checks/clearance for working with a vulnerable population.

CRITICAL SKILLS, ABILITIES, & EXPERTISE:

Physical Requirements: These physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of the position. Talking, standing, sitting, squatting, kneeling, use of fingers, arms, hands, legs, walking, stretching/reaching, handling, grasping, climbing, bending at the waist, turning, balancing, pushing, pulling, use of depth perception, carrying and lifting (up to 65 lbs.), and driving are constant. Good eyesight (correctable) and hearing (correctable) are essential.

Equipment: Computer, facsimile, copy machine, calculator, multi-line phone and other small office and instructional equipment and vehicle.

Skills & Expertise: Ability to work with limited direction. Knowledge of organizational methods. Skills in completing work with a high degree of accuracy. Ability to effectively communicate orally and in writing. Ability to establish effective working relationships with people, including supervisors, peers, vendors, and clients. Ability to compile data from various sources, departments, etc. Ability to type at least 40 words per minute. Expertise in the operations of Word processing, and Window based applications. Ability to use and operate personal computer and general office equipment. Skills in customer service and dealing with difficult individuals. Ability to work in a multi-task environment. Ability to maintain client and staff confidentiality.

ENVIRONMENTAL JOB FACTORS:

Job Location: Position is located within Hernando, Sumter, Lake or Polk County. However, this position can be relocated at any given time for any reason with sufficient notice provided. Ongoing travel to other facilities and/or counties may be required.

Work Environment: While performing the responsibilities of the position, these work environment characteristics are representative of the environment the employee will encounter. Office environment and working with others frequently. Working with office equipment is frequent. Occasionally outdoors and operates a vehicle.
