MDS Nurse

Job Summary:

We are seeking a highly skilled and experienced MDS Nurse to join our team at Villa Serena Healthcare Center in Long Beach, California. As an MDS Nurse, you will play a vital role in developing treatment plans for residents that comply with MDS guidelines. If you are a detail-oriented professional with excellent communication skills, we encourage you to apply.

Responsibilities and Job Functions:

  • Conducts and coordinates the development and completion of the Minimum Data Set (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment.

  • Maintains MDS schedule within stated MDS time frames and informs social workers of pending MDS assessments.

  • Generates and distributes monthly MDS calendar.

  • Creates new assessments according to MDS schedule.

  • Submits the MDS report in a timely fashion to meet established reporting deadlines.

  • Ensures the accuracy of the MDS report.

  • Maintains all standards established by accrediting agencies and participates in the planning of constructive change.

Requirements:

  • Must possess a Nursing degree from an accredited nursing program.

  • Must be licensed as a Registered Nurse in the State of California.

  • Must possess MDS Certification from an accredited program.

  • MDS experience in a Long-Term care setting.
