Signature Home Health is seeking a compassionate and skilled Physical Therapist to provide in-home rehabilitation services that help patients regain mobility, improve strength, and maintain independence. Key Responsibilities
  • Develop and implement individualized treatment plans.
  • Perform patient assessments, including mobility, gait, strength, and range of motion evaluations.
  • Provide physical therapy services in accordance with physician-directed care plans.
  • Educate patients, families, and caregivers on therapy goals, exercises, and safety measures.
  • Monitor patient progress and maintain accurate clinical documentation.
  • Collaborate with physicians and interdisciplinary healthcare teams.
  • Participate in care conferences and discharge planning.
  • Supervise Physical Therapy Assistants and home health aides as required.
  • Identify additional patient care needs and coordinate appropriate referrals.
Qualifications
  • Current Oregon Physical Therapist license.
