Job description

Social Services Director Opportunity at Countryside Meadows

Avon, Indiana

Experience in a long-term care, skilled nursing, rehabilitation, or healthcare setting preferred

The Social Services Director is responsible for assessing residents' psychosocial needs and coordinating services that promote the highest practicable physical, mental, and psychosocial well-being of each resident. This role works closely with residents, families, and the interdisciplinary team to support quality of life, discharge planning, and resident advocacy while fostering a compassionate, resident-centered environment.

Skills Needed:

Resident Advocacy & Psychosocial Support: Assess residents' psychosocial needs, develop appropriate interventions, and provide emotional support to residents and families throughout their stay.

Care Planning & Collaboration: Participate in interdisciplinary care planning, review resident needs, and collaborate with nursing and other departments to ensure individualized, person-centered care.

Communication & Conflict Resolution: Effectively communicate with residents, families, ombudsmen, and the care team. Mediate resident and family concerns, including roommate conflicts, room changes, and other sensitive situations with professionalism and compassion.

Discharge Planning: Coordinate discharge planning, identify potential social or economic barriers to a successful discharge, and connect residents and families with appropriate community resources and referrals.

Organization & Documentation: Maintain accurate documentation, ensure compliance with state and federal regulations, and manage multiple priorities in a fast-paced long-term care environment.

Requirements:

  • Bachelor's degree in Social Work, Human Services, or a related field preferred.
  • Experience in a long-term care, skilled nursing, rehabilitation, or healthcare setting preferred.
  • Experience working with geriatric residents, including those with dementia or behavioral health needs, preferred.
  • Strong communication, conflict resolution, organizational, and interpersonal skills.
  • Knowledge of discharge planning, community resources, and resident rights preferred.
