  2. Develop and implement individualized treatment plans that incorporate clinical guidelines, patient preferences, and shared decision-making, including prescribing medications, performing procedures within granted privileges, and monitoring treatment outcomes.
  3. Coordinate care across the healthcare continuum by collaborating with specialists, behavioral health providers, pharmacists, nurses, care coordinators, community resources, and other interdisciplinary team members to promote continuity and optimize patient outcomes.
  4. Promote population health and quality improvement by addressing preventive care, chronic disease management, care gaps, evidence-based quality measures, and organizational quality initiatives.
  5. Educate and empower patients and families by providing culturally responsive education regarding diagnoses, treatment options, medications, preventive care, and healthy lifestyle behaviors to support informed decision-making and self-management.
  6. Maintain timely, accurate, and complete clinical documentation in the Electronic Health Record, including documentation, coding, orders, patient communications, and other required records in accordance with organizational policies and regulatory standards.
