Core Health Provider Services, Inc has an employment opportunity as a Direct Support Professional at our residential group homes. We are looking for individuals who have a sincere desire to help others. Our mission is to serve adults with special needs, by providing specialized services including assisting with ADL's, tracking goals, and community outings. Supports are individually planned and tailored to meet the specific needs of the individuals we serve. This may include working at multiple homes, locations, and departments.
Position Details:

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Adheres to assigned schedules and reports to duty for the hours assigned and implements planned schedules and activities
  • Supports in activities and companionship services for daily living, such as cooking, bathing, dressing, grooming, feeding, toileting, transferring, and other similar tasks
  • Accompanies and transports to participate in visits for recreation, medical and dental care appointments, therapies, personal shopping and other community activities
  • Implement ISP goals
  • Assists in creating a physical environment that lends to progress including house cleaning and laundry
  • Other duties as assigned
Required Qualifications :

The successful candidate must:
  • Be motivated and self-directed
  • Have a high school diploma or GED (proof will be required)
  • Have dependable transportation, valid driver’s license, and valid car insurance
  • Be able to satisfactorily pass a criminal background check
  • Provide a clean 7 year driving record, with no suspensions due to moving violations in past 3 years
  • Must obtain a TB test
  • Current CPR/First Aid certification
  • Current physical
  • Proven experience as personal care assistant
  • Well-organized and reliable
  • Excellent communication and people skills
  • Positive and compassionate attitude
  • Committed to health & safety
Other Requirements: MUST complete required training education certification to continue professional development.
