Job Description

Clinical Pharmacist at Providence Santa Rosa Memorial Hospital in Santa Rosa, CA. This position is Per Diem and will work 10-hour, Variable shifts.

The clinical pharmacist I has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.

Required Qualifications:
  • PharmD in Pharmacy Doctorate Or
  • Bachelor's Degree in Pharmacy
  • Upon hire: California Pharmacist Registered
  • Upon hire: Basic Life Support (BLS) - where applicable
Preferred Qualifications:
  • Acute care hospital pharmacy experience, 1 year or equivalent experience.
  • Completion of ASHP-accredited PGY1 residency.
