This position is fully remote, offering flexibility to accommodate varying schedules to meet client needs. While most services can be provided online, occasional evening and weekend hours may be required to ensure optimal access for clients. As a Licensed Clinical Social Worker at Second Chance Marriage and Family, Inc., you will:

• Conduct thorough psychosocial assessments to diagnose and craft individualized treatment plans.
• Utilize evidence-based therapeutic interventions such as solution-focused therapy, behavioral activation, and motivational interviewing to support patient progress.
• Monitor client progress methodically by utilizing specialized metrics and adaptive feedback tools, ensuring treatment goals are met or adjusted as necessary.
• Collaborate with multidisciplinary teams including primary care physicians and other behavioral health providers to cultivate a seamless care continuum for patients.
• Assist patients in managing psychotropic medications, focusing on adherence, side effects, and treatment effectiveness in line with medical guidance.

Your clients will primarily include individuals and families from various backgrounds seeking support for mental health challenges. This may encompass areas such as anxiety, depression, relationship issues, and family dynamics. The ability to address culturally diverse perspectives will be vital in providing effective care and establishing trust with patients.

To qualify for this role, the candidate must possess:

• A Master's degree in Social Work from an accredited institution.
• A current, valid, and unrestricted license to practice independently in mental health as a Licensed Clinical Social Worker (LCSW) within New Jersey.
• A minimum of one year of experience providing therapy in an independent capacity.

While not required, experience in community health settings and knowledge of integrated care models will be highly valued. Familiarity with electronic health record (EHR) systems and other telehealth platforms will be advantageous in facilitating effective patient management.
