Second Chance Marriage and Family, Inc. is seeking a full-time Licensed Clinical Social Worker (LCSW) to join our dedicated team. The LCSW role is designed to be fully remote, allowing for flexibility and work-life balance. While the primary work will be conducted online, you will have the opportunity to engage in occasional in-person sessions at our community health clinic located in Hempstead, New York, as needed. • Conduct comprehensive psychosocial evaluations to accurately diagnose and assess the needs of patients individually.
• Develop and implement tailored treatment plans that address the unique requirements of each patient, utilizing evidence-based approaches such as solution-focused therapy and motivational interviewing.
• Regularly monitor patient responses to therapy, adjusting treatment strategies as necessary to enhance their journey towards achieving their mental health goals.
• Collaborate with multidisciplinary teams, including primary care providers, psychiatrists, and other mental health professionals, to ensure an integrated approach to patient care.
• Oversee the administration of psychotropic medications as prescribed, focusing on patient adherence and potential side effects to improve treatment efficacy.
• Engage in ongoing documentation and reporting of patient progress, using established electronic health record (EHR) systems to maintain compliance and enhance interdepartmental communication.

Our LCSW will work with a diverse array of patients, including children, couples, and families facing challenges such as relationship issues, anxiety, depression, trauma, and more. We are particularly focused on supporting those from marginalized communities who experience barriers in accessing traditional health services. By understanding and addressing the unique needs of each client, you will contribute to fostering resilience and coping strategies that can lead to meaningful improvements in their lives.

• A Master's degree in Social Work from an accredited institution.
• A current and valid license to practice independently as a Licensed Clinical Social Worker (LCSW) in New York.
• At least one year of experience as a licensed therapist with a focus on mental health.

• Experience using innovative therapeutic modalities and a familiarity with telehealth practices.
• Proven ability to work collaboratively with multidisciplinary teams to enhance patient care.
• Knowledge of community resources and how to effectively connect patients with these supports.

The salary for this position ranges from USD 80,000 to 100,000 annually, commensurate with experience and qualifications. In addition to a competitive salary, we offer a variety of benefits including:
