Description

American Traveler is hiring a travel RN Care Manager for an acute care hospital case management position requiring at least 2 years of RN experience and an active NC or compact RN license.

Details
  • Works in a hospital-based Care Management department covering Cardiology, Nephrology, Trauma Surgery, Ortho Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, and ICU/Stepdown units
  • Manages a caseload of acutely ill patients with complex and varying medical needs across units of 20–25 beds
  • Responsibilities span case management, utilization review, and discharge planning
  • Uses Epic EMR for assessments and care plan documentation
  • Day shift, 8:30 AM – 5:00 PM, Monday through Friday
  • No on-call, weekend, or holiday requirements
Requirements
  • Active RN license in North Carolina or a compact license recognized in NC
  • Minimum 2 years of RN experience in an acute care or hospital setting
  • Experience as a case manager, care manager, or discharge planner in a hospital setting
  • Nursing Diploma or ADN/ASN from an accredited school of nursing (BSN or MSN preferred)
  • Strong assessment and critical thinking skills
  • Epic EMR experience
  • Two professional references required for consideration
Additional Information
  • Conducts comprehensive patient assessments and develops individualized care plans in Epic per departmental guidelines
  • Participates in daily Care Management Touchpoint meetings and weekly Complex Care Meetings (CCM) to review patient progression and problem-solve complex cases
  • Collaborates with an interdisciplinary team including Social Workers, Utilization Managers, and Care Management Assistants to address discharge barriers and psychosocial concerns
  • Coordinates family meetings, community resource referrals, and post-acute care transitions
  • Monitors observation patients throughout the day to ensure appropriate progression of care and discharge readiness
  • Experience in educational or academic teaching hospital environments is a plus
  • Background check and drug screen are required prior to start; candidates should be prepared to report for drug screening promptly upon commitment
