The Physical Therapist will provide onsite services within the home care setting, transitioning rehabilitation and therapeutic care directly to patients' residences. As a Physical Therapist in our home care program, you will work with a diverse range of patients, including older adults managing chronic conditions, post-surgical patients requiring rehabilitation, and individuals recovering from injuries. You will apply your expertise to facilitate the healing process, enabling patients to regain independence and improve their quality of life in their own environments.

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Active state licensure in Physical Therapy, with a commitment to maintaining good standing with relevant regulatory boards.
• Basic Life Support (BLS) certification required.
• Exceptional verbal and written communication skills, with proficiency in electronic health record (EHR) systems.

The ideal candidate will have at least two years of hands-on experience as a physical therapist, specifically in home health or outpatient settings. Experience working collaboratively within an interdisciplinary healthcare team is desirable.

This position offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
