  • Minimum 2 years acute care hospital Case Management experience
  • Strong background in Care Coordination & Discharge Planning
  • Hospital experience required
  • Active California RN License (pending accepted, must be active by start date)
  • Must be open to weekend rotation & department needs

Preferred:

  • Strong communication and multidisciplinary coordination skills
  • Previous travel experience preferred but not required
