We are a fast-growing Minnesota Home Healthcare provider and we are searching for IHS (In-Home Support) Workers to join our team.

Job Description – Individualized Home Supports (IHS) with Training Specialist (245D) Position Summary Safeway Home Healthcare is seeking experienced, dependable, and compassionate Direct Support Professionals to provide Individualized Home Supports (IHS) with Training under Minnesota DHS 245D. This role focuses on teaching, coaching, and supporting individuals to increase their independence while helping them achieve goals identified in their support plans. Staff provide hands-on teaching and guidance rather than completing tasks for the person whenever possible. Typical Schedule Most positions include 2–3 permanent clients with consistent weekly schedules. Typical client schedules include:
  • Monday–Friday
  • Morning shifts (approximately 8:00 AM–12:00 PM) or
  • Afternoon shifts (approximately 12:00 PM–5:00 PM)
  • Limited evening shifts
  • Minimal weekend availability
In addition to primary clients, staff will also:
  • Complete meet-and-greets with additional clients.
  • Be trained on backup clients to provide coverage when needed.
  • Occasionally assist with temporary coverage assignments.
Qualifications Preferred candidates will have:
  • 3–5 years of experience providing direct support services in a residential, facility, or community-based setting.
  • Experience supporting individuals with disabilities, mental health needs, or other complex support needs.
  • Strong communication and relationship-building skills.
  • Excellent time management and documentation skills.
  • Reliable transportation and a valid driver's license, as transportation is frequently a component of services.
  • Ability to work independently while collaborating with supervisors and interdisciplinary teams.

Transportation Transportation may be provided when it supports an authorized IHS goal or community activity. Transportation is a component of the service and is not provided as a stand-alone transportation service. Documentation Responsibilities Staff are expected to:
  • Complete electronic documentation in Therap.
  • Record progress toward identified goals.
  • Document services accurately and promptly.
  • Follow Electronic Visit Verification (EVV) requirements.
  • Communicate significant changes or concerns to supervisors.
