Job description

Minimum Qualifications:

  • Graduation from a Licensed Vocational Nurse program and licensure/permit as a vocational nurse in the state of Texas. 
  • No experience required. 
  • Current BLS required. 

Job Description:

To provide direct nursing care to an assigned group of patients and perform nursing procedures within the scope of their education and demonstrated abilities. Under the direction of the Program Manager and licensed healthcare providers, The Vocational Nurse (LVN) provides direct nursing services and care coordination to individuals enrolled in the Ryan White HIV/AIDS Program. The position supports client engagement in care, treatment adherence, health education, care plan implementation, referral coordination, and compliance with Ryan White Program standards. Vocational Nurse functions as an integral member of the multidisciplinary  care team to improve client outcomes, reduce barriers to care, and promote retention in HIV medical care. 
 

Job Duties

  • Provides direct nursing care and clinical support to patients within the scope of vocational nursing practice, ensuring safe, effective, and patient-centered care
     
  • Assists with client assessments, care plan implementation, treatment adherence education, and ongoing monitoring to support engagement in HIV medical care and positive health outcomes.
     
  • Coordinates and facilitates referrals to medical, behavioral health, and supportive services; collaborates with multidisciplinary team members to address barriers to care and promote continuity of services. 
     
  • Maintains accurate and timely clinical documentation, supports quality improvement activities, and ensures compliance with Ryan White Program standards, UTMB policies, and regulatory requirements.
     
  • Participants in client outreach, retention-in-care-efforts, eligibility support activities, and other program initiatives designed to improve access to care and client outcomes.
     

Salary Range:
Commensurate with experience
 

Equipment: Standard patient care supplies, equipment, and mechanical devices.
 

Working Enviroment: Standard hospital and ambulatory setting
