Quality Assurance Nurse / Compliance Coordinator – Keep Home Health Excellence Measurable

We provide medical care, in‑home support, hospice and palliative care, cancer navigation, and 200+ other free services to qualifying seniors. Every service is delivered at no cost, regardless of insurance status.

Role Overview:

As QA Nurse / Compliance Coordinator, you will own clinical quality, regulatory compliance, and survey readiness for our home health program. You will conduct chart audits, drive performance‑improvement initiatives, and ensure we are always survey‑ready.

Key Responsibilities:
  • Conduct OASIS chart audits and clinical‑record reviews to identify quality opportunities
  • Lead performance‑improvement projects targeting specific Star Ratings, HHCAHPS, and outcome metrics
  • Prepare the agency for state and CMS surveys; manage corrective‑action plans when needed
  • Train clinical staff on documentation standards, infection control, and patient safety
  • Track and report on quality indicators monthly to the leadership team
  • Investigate complaints and adverse events; document findings and follow‑up
Qualifications:
  • Current CA RN license + BSN preferred
  • 3+ years home health nursing with at least 1 year in QA or QAPI
  • OASIS‑C certification (COS‑C) strongly preferred
  • Familiarity with CMS Conditions of Participation, Star Ratings, HHCAHPS
  • Strong attention to detail and data‑analysis skills
