Job description

Job Summary

The Rural Health Liaison conducts outreach activities and serves as a resource for members, providers, and community-based organizations (CBOs) to address the diverse health needs of rural populations. This role leads outreach initiatives, facilitates education and support programs, and contributes to policy development aimed at promoting health equity and enhancing health outcomes in rural communities.

Essential Functions
  • Engage with members, providers, and CBOs and other stakeholders to share information, resources, and provide education on health access and services.
  • Provide proactive, high-level account development and support to drive overall satisfaction and engagement of key rural health partners.
  • Handle, refer, or delegate complex or non-routine questions, issues, and problems to appropriate resources and ensure they are resolved in a timely manner (≤30 days).
  • Maintain organized files and document all interactions in CRM.
  • Support recruitment and contracting by building relationships with providers, hospitals, and ancillary providers.
  • Promote access to care by connecting rural community members to health-care services, benefits, programs, and resources.
  • Conduct outreach activities, including presentations and educational materials, to increase awareness of health issues and promote healthy behaviors.
  • Collect and analyze data on rural health needs and challenges to identify outreach opportunities.
  • Provide input for policy development and support implementation of innovative solutions for rural communities.
  • Facilitate, coordinate, and deliver cultural-competency training for CareSource staff.
  • Provide resources to providers and CBOs to enhance care and health literacy.
  • Organize and host community town-halls, advisory groups, listening sessions, and other events.
  • Build relationships among rural community-based organizations, stakeholders, and providers.
  • Collaborate with the Managed Care Organization (MCO) to address social determinants of health and integrate wellness and holistic care.
  • Promote MCO plan services and benefits to improve health equity.
  • Develop comprehensive success metrics that demonstrate impact and value of Rural Health initiatives.
  • Perform other job-related instructions as requested.
Education and Experience
  • Bachelor’s degree or equivalent experience (preferred social work or public health).
  • Minimum of three (3) years of health care, public health, or community-based organization experience relevant to the role.
  • Familiarity with rural health communities, rural health disparities, and Medicaid knowledge preferred.
Competencies, Knowledge and Skills
  • Proficient in Microsoft Office (Word, PowerPoint, Excel, Access).
  • Fluency in English; Spanish language preferred.
  • Sensitivity to and experience working within diverse cultures.
  • Ability to work independently and as part of a team.
  • Knowledge of community and state support resources.
  • Capacity to manage situations involving multiple interests and stakeholders.
  • Strong planning, organizational, and multi-tasking skills.
  • Excellent interpersonal, listening, and relationship-building skills.
  • Research and analytical skills.
  • Program development experience.
  • Excellent written, verbal, and presentation skills.
Licensure and Certification

Preferred: Community Health Worker certification.

Driver and Insurance Requirements

The position is conditional on a valid driver’s license, vehicle, and verifiable insurance. The offer will be withdrawn if the license record is unacceptable, and employment may be terminated if problems arise after hiring.

Vaccination Requirement

Influenza vaccination is required for this position. Employees must provide proof of vaccination during influenza season (October 1 – March 31) and have verification in the employee’s file.

Working Conditions

This mobile position requires regular travel to various sites, including homes, offices, and public spaces. Employees may be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods. Must reside in the assigned territory; travel may exceed 50 percent of time.

Compensation

Salary range: $62,700 – $100,400. Compensation is based on education, training, experience, scope, and complexity of the role. Additional bonus may be available based on company and individual performance. Benefit package is comprehensive and includes health, dental, vision, and retirement options.
