Position Summary
The Intake RN will be responsible for conducting initial assessments of new clients, developing individualized care plans, and ensuring smooth admission into our healthcare services. The Intake RN serves as the primary clinical point of contact during the onboarding process and works closely with physicians, caregivers, and family members to ensure continuity of care.
Key Responsibilities
  • Conduct comprehensive initial assessments of new clients, including medical history, functional status, and home environment.
  • Develop, document, and update individualized care plans based on client needs and physician orders.
  • Educate clients and families on care plans, medications, and home healthcare processes.
  • Ensure all intake documentation is accurate, timely, and compliant with state and federal regulations.
  • Coordinate with physicians, care team members, and referral sources to facilitate seamless client admission.
  • Assess eligibility for programs (Veterans, GAPP, CCSP, Medicaid/Medicare, private pay).
  • Monitor client progress during the transition into care and provide follow-up as needed.
  • Maintain confidentiality of patient records and comply with HIPAA guidelines.

Qualifications
  • Current and valid Registered Nurse (RN) license in the state of Vermont.
  • Minimum 1 year of clinical nursing experience (home healthcare or intake/admissions preferred).
  • Strong assessment, documentation, and communication skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Familiarity with state/federal home health regulations a plus.
  • CPR Certification (current).

Schedule & Compensation
  • Flexible scheduling available (part-time or full-time).
