What You'll Do

  • Provide routine and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing settings.
  • Complete nursing assessments, symptom management, medication education, and comfort-focused interventions.
  • Support patients and families through education, advocacy, and compassionate communication.
  • Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
  • Complete accurate, timely EMR documentation.

What You'll Bring

  • Active Minnesota Registered Nurse (RN) license.
  • Hospice experience preferred, or experience in home health, oncology, ICU, palliative care, med-surg, or another clinical setting with end-of-life care exposure.
  • Excellent assessment, communication, and time-management skills.
  • Ability to work independently in a field-based role.
  • Reliable transportation and a valid driver's license.
