Description:
The Care Manager RN provides professional, comprehensive, patient-centered care management services for at-risk patients in an acute care environment. This role focuses on coordinating care across the continuum to ensure quality outcomes, appropriate resource utilization, and safe discharge planning.
Key responsibilities include:

โข Coordination of care across multidisciplinary teams

Qualifications:
โข Associate Degree in Nursing (ADN) or nursing diploma
โข Active Washington State Registered Nurse (RN) license
โข Minimum of 2 years acute care experience in a Medical-Surgical or inpatient setting

โข Bachelor's Degree in Nursing (BSN) or higher (within 3 years of hire)
โข National certification in area of specialty
โข 1 year experience in care management or utilization review OR
โข Successful completion of a Case Management Orientation Program or TIPS program
Compensation:
USD $85,494 to $160,486
