What is an RN First Assistant?

RN First Assistants (Registered Nurse First Assistants) are specialized registered nurses who work alongside surgeons and surgical teams during operations. They play a critical role in patient care by assisting with surgical procedures, providing direct patient care before, during, and after surgery, and helping to ensure the best possible outcomes. RNFAs are trained in advanced surgical techniques, such as suturing, controlling bleeding, and handling tissues, and they must have additional education and certification beyond standard registered nurse credentials.

What are the key skills and qualifications needed to thrive as an RN First Assistant?

To thrive as an RN First Assistant, you need advanced clinical knowledge in perioperative nursing, surgical procedures, and patient assessment, typically supported by RN licensure, perioperative experience, and CNOR or RNFA certification. Familiarity with surgical instruments, sterile technique, and electronic health records is essential for this role. Strong communication, critical thinking, and teamwork skills help you collaborate effectively with surgical teams and respond quickly in high-pressure situations. These skills and qualifications are crucial for ensuring patient safety, reducing complications, and supporting optimal surgical outcomes.

What are the most common challenges faced by an RN First Assistant in the operating room?

As an RN First Assistant, one of the most common challenges is balancing direct surgical responsibilities with maintaining patient safety and effective communication within a fast-paced environment. You'll often need to anticipate the needs of the surgical team and adapt quickly to unexpected situations during procedures. Collaboration with surgeons, anesthesiologists, and other OR staff is crucial, and handling stressful or high-pressure moments with composure is a key part of the role. Continuous learning and staying updated with surgical techniques also help in overcoming these challenges.

Certified Nurse Midwife (CNM)

Compensation: $115,000 โ€“ $145,000

Job Type: Full-Time | On-Site

Location: Rutland, Vermont

We have partnered closely with a prominent regional healthcare system to secure a full-time, dedicated clinical professional for their women's health services division. We are currently seeking a passionate, full-scope Certified Nurse Midwife (CNM) to join a thriving, hospital-owned OB/GYN practice in Rutland, Vermont.

As an integral part of a dynamic collaborative care teamโ€”comprising four OB/GYN physicians and a Women's Health Nurse Practitionerโ€”you will provide exceptional, patient-centered, evidence-based care while supporting normal physiological changes across the reproductive lifespan. The hospital features an exceptionally autonomous nursing team, respiratory therapists, and pediatricians, facilitating approximately 300 births annually.

Responsibilities & Call Schedule:

  • On-Call Structure: Take first call for OB and Gyn patients. Call shifts are typically 24-hour shifts, structured predictably as one day a week and one weekend a month. You will have a dedicated office located on the same floor as the Labor & Delivery unit for your exclusive use when on call.
  • Provide full-scope midwifery care, managing the wellness, reproductive, and gynecological health of women from adolescence through menopause.
  • Deliver autonomous and collaborative intrapartum care, managing labor and delivery at the adjacent medical center.
  • Collaborate closely with the hospital's highly autonomous nursing team, respiratory therapists, and pediatricians to facilitate roughly 300 births each year.
  • Maintain highly collaborative, evidence-based practices that focus on patient safety, quality improvement initiatives, and comprehensive patient education.
  • Complete accurate, timely, and compliant electronic charting and documentation in full alignment with hospital and regulatory standards.

Qualifications:

  • Active, unrestricted Registered Nurse (RN) and Certified Nurse Midwife (CNM) licensure per the Vermont State Board of Nursing (or immediate eligibility to obtain prior to hire).
  • Current national certification by the American Midwifery Certification Board (AMCB). Active Basic Life Support (BLS) through the American Heart Association and Neonatal Resuscitation Program (NRP) certifications are required.
  • Active DEA registration number (or the ability to obtain immediately upon hire).

Preferred:

  • Minimum of 2 years of professional CNM experience
  • Proven experience with ultrasound assessment and interpretation
  • Hands-on proficiency with GYN procedures (including IUD/Nexplanon insertions, minor I&Ds, and colposcopy)
  • Prior clinical experience serving as a first assist at C-sections
  • Professional background operating comfortably in both outpatient clinics and hospital-based acute labor and delivery settings

Benefits:

  • Competitive salary
  • Signing bonus
  • Relocation assistance
  • Educational loan assistance
  • Malpractice coverage provided
  • Annual CME funding
  • 403(b) retirement plan with employer match
  • Comprehensive health, dental, vision, life, and disability insurance
  • Combined Time Off (CTO)
  • College Savings Account option

Note on Compensation & Hiring Requirements: Final base salary and financial support packages will be verified and determined by the organization based on your documented years of midwifery practice, specialized procedural skills, and historical training. All job offers are strictly contingent upon the successful completion of standard pre-employment screenings, including a criminal background check, drug screening, health/immunization verification, and full facility credentialing.

Note on Employment Status: The comprehensive executive benefits package and incentive structures detailed above are explicitly offered for full-time positions. Eligible benefits may vary or be prorated for alternative employment arrangements.
