Responsibilities:
  • Provide high-quality, person-centered care to residents in our skilled nursing facility, focusing on their physical, emotional, social, and spiritual needs

  • Assess, plan, implement, and evaluate individualized care plans, collaborating with other healthcare professionals to ensure optimal outcomes

  • Administer medications, treatments, and therapies as prescribed by physicians

  • Monitor and report changes in resident condition to ensure timely interventions
