In the role of Registered Nurse Case Manager, you'll be entrusted with providing 1:1, whole-person care to patients and families in their homes. QualificationsRegistered Nurse Case Manager Requirements & Responsibilities:
- Associates, Bachelors, or Masters of an accredited nursing program (RN)

- Develop individualized care plans with the patient and family, updating as needs evolve
- Administer medications and treatments as prescribed
