Location: 500 Upper Chesapeake Dr, Bel Air, MD 21014
Duration: 13 Weeks | Start: 05/04/2026 or ASAP
Shift Details:
  • Day Shift: Monday-Friday
  • 08:00 AM - 04:30 PM

Job Summary:
Seeking an experienced RN Case Manager (or SW preferred) with inpatient case management experience to handle complex discharge planning, legal coordination, and patient support services.
Responsibilities:
  • Manage ~15 inpatient cases daily
  • Coordinate safe discharge planning, placement, and home care services
  • Handle complex cases including locating family/support systems
  • Work on guardianships, capacity evaluations, and certificates of competency
  • Collaborate with legal teams and attend court hearings with patients
  • Address social, medical, and placement-related needs

Requirements:
  • RN with Inpatient Case Management experience (SW preferred)
  • Strong experience in discharge planning and care coordination
