Registered Nurse - Home Health RN / Community Nurse

As a Home Health RN / Public Health Nurse, you'll provide one-on-one care to patients in their homes, helping them feel safe, supported, and empowered in their health journey.

Qualifications:
  • Current RN license in Iowa
  • At least 1 year of nursing experience (home health or community health a plus)
  • BLS certification (American Heart Association)
  • Valid driver's license and dependable transportation
  • Bachelor's degree preferred, or equivalent nursing experience
  • Organized, self-motivated, and comfortable working independently
  • Strong communication and EMR documentation skills
  • Own a personal cell phone
What You Will Do:
  • Visit patients in their homes to provide skilled nursing care and emotional support
  • Develop and manage personalized care plans based on each patient's needs
  • Administer medications, monitor progress, and educate patients and families
  • Coordinate care with physicians and other providers
  • Supervise Home Health Aides and ensure care standards are met
  • Support community health initiatives and health promotion events
  • Participate in on-call rotation (typically 1-2 weekends/month)
  • Document all care using an electronic health record system
