Job description

Overview

Data Abstractors enter clinical data from patient charts daily into the required systems and produces reports from as requested.

Responsibilities
  • Obtains clinical data from patient charts and enters daily for the previous 24 hour billing period for each patient.
  • Completes the data collection daily and exports files as necessary.
  • Runs various reports daily and as required at intervals during each week.
  • Provides some administrative support for the practice as necessary.
  • Travels to multiple sites as necessary.
  • Presents a positive, professional appearance and conveys a professional demeanor in the performance of assigned duties
  • Maintains strict confidentiality in accordance with HIPAA regulations and Company policy.
  • Responsible to support data collection and tracking quality metrics. 
  • Assist with preparation of presentations, summaries and help maintain dashboard.
  • Collaborate with multidisciplinary teams to ensure alignment with quality goals and follow up on action items.
  • Support PDSA cycles, development and dissemination of protocols, guidelines, or quality bundles as needed.
  • Assist with preliminary analysis related to clinical outcomes and quality metrics.
  • Participate in quality meetings and contribute to discussion of performance trends and improvement opportunities.
  • Coordinate follow-up appointments for discharged patients from Women's Hospital and other referring facilities as appropriate.
  • Communicate with patients, providers, and hospital staff to ensure timely scheduling and continuity of care.
  • Verify receipt of discharge documentation and ensure all required records are available prior to scheduling.
  • Maintain accurate referral tracking logs and appointment documentation.
  • Fax discharge summaries daily to OB offices and maintain detailed tracking logs of all faxes sent.
  • Follow up with OB offices as needed to confirm receipt of records and ensure continuity of care.
  • Maintain and update transport tracking Excel spreadsheets accurately and timely.
  • Communicate and coordinate with Neonatology staff as needed regarding transport-related information and follow-up items.
  • Provide excellent customer service and professional communication to patients and healthcare partners.
  • Performs other duties as assigned.
Qualifications

Education:

  • High school diploma or equivalent. Associate's degree preferred.

Experience Industry: Healthcare

Experience:

  • Six months to one year related experience.
  • Equivalent combination of relevant education and experience will be considered.

Skills/Abilities:

  • Computer savvy with excellent typing skills.
