Coverage Territory: Bloomington, Pontiac, and surrounding areas

Schedule: 8 AMโ€“8 PM, alternating 4-day and 3-day weeks including weekends in rotation

What You'll Do

As an RN Case Manager, you'll be the clinical anchor for your patients and their families โ€” coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:

  • Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
  • Building and adjusting personalized care plans in collaboration with physicians and care teams
