This full-time position will require you to work onsite within our home care facility, providing a supportive and dynamic atmosphere where our team collaborates closely with other healthcare professionals. As a Physical Therapist at Revive Motion Clinic, your daily responsibilities will include, but are not limited to:

• Conducting comprehensive assessments of patients with functional limitations and disabilities to develop individualized treatment plans.
• Implementing physical therapy interventions and modifications based on patient progress, ensuring compliance with established guidelines.
• Documenting patient evaluations, treatments, and outcomes diligently, adhering to all legal and regulatory standards.
• Collaborating with interdisciplinary team members to optimize each patient's continuum of care, discussing changes with physicians as necessary.
• Providing education to patients and their families on treatment goals and techniques, facilitating informed and empowered health decisions.

Familiarity with electronic health record (EHR) systems and prior involvement in community outreach or program development activities would be advantageous.
