Licensed Practical Nurse (LPN) - Part Time - Nights

The primary functions of an LPN in assisting with assessment, planning, implementing and evaluating the care of all patients, regardless of age, during his/her shift. Responsible for meeting the established standards of skilled nursing practice. The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients. Effectively provides Person Centered care and services.


Roles and Responsibilities:

  • Administers medications and treatments under the supervision of a Registered Professional Nurse in accordance with Facility Policy.
  • Taking the patient's vital signs such as respiration, temperature, blood pressure, and heart rate and reporting changes in the patient's /residents conditions
  • Practices ethically and professionally within their scope; advocates for patient
  • Maintains clinical competence specific to their care area including organization required education within stated timeframe
  • Coordinates care with interdisciplinary team
  • Documents appropriately and thoroughly in EMR/Point Click Care
  • Keeps other nursing personnel and responsible RN informed regarding changes in condition of patients/ residents and / or other pertinent observations
  • Participates in unit councils and staff meetings.

Required Skills and Experience:

  • Graduate of a Nursing Program
  • Current NYS LPN License
  • CPR/BLS

Preferred Skills and Experience:

  • Prior patient care and customer service experience

Physical Requirements:

  • May require lifting (up to 50 lbs.), may require sitting or standing for long periods. Must be able to physically operate the equipment required for the job.

Pay Range Disclosure:

  • $26.15 to $37.00 per hour
