Description
- Assess patient conditions in both inpatient and outpatient environments and create tailored treatment plans
- Deliver physical therapy interventions that prioritize mobility, strength, balance, and functional recovery
- Team up with nurses, physicians, case managers, and other rehabilitation professionals to enhance outcomes
- Guide patients and their families through therapeutic objectives, discharge planning, and home exercise regimens
- Document evaluations, treatment processes, and progress, in line with facility and regulatory standards
- Engage in care team meetings and interdisciplinary forums to facilitate coordinated patient care
Education
- Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
Certifications
- State licensure as a Physical Therapist
- CPR certification preferred
- Specialty certifications (e.g., neurological, geriatric) considered beneficial but not necessary

- Prior experience strongly preferred
